The McCray Agency, which represented Tate prior to her death, confirmed that she died "peacefully" after complications from "a rare form of Charcot-Marie-Tooth disease" on their Instagram Stories.

Charcot-Marie-Tooth disease is a group of inherited conditions that damage the peripheral nerves, particularly in the arms and legs, per the NHS.

In the statement, the agency noted that her family "requests privacy as they deal with this sudden and shocking loss."

They also noted that Tate's family asked that donations in her memory be made to the Charcot-Marie-Tooth Association, "in lieu of flowers."

The McCray Agency did not immediately respond to PEOPLE's request for comment.

Tate was born in Nashville, Tenn., where she grew up and went to school, according to her obituary. She earned a bachelor's degree in business and worked as an aspiring actress — all with the desire of helping to "change the world."

"Isabelle was full of fire, a fighter, never once making excuses for the fact that she might have a disability relative to others," her obituary read.

It continued: "She was also quite musically inclined, often spending hours writing and recording songs with friends and even publishing a few. What she loved the most though was spending time with family and friends, always the life of the party."

She isn't the only public figure to have been diagnosed with Charcot-Marie-Tooth. In 2010, screen icon Julie Newmar spoke with WABC-TV about being diagnosed with the disease.

More recently, country music singer Alan Jackson spoke candidly about living with Charcot-Marie-Tooth, sharing in a post on X last year that his illness made it challenging for him to perform and was "getting worse." He admitted that he was thinking of "calling it quits" before he's "unable to do the job "like I want to."

9-1-1: Nashville airs Thursdays at 9 p.m. ET/PT on ABC.
